Lung Transplant and Bronchoscopy: How to Diagnose a Rejection

What is a Bronchoscopy?


Patients that have received a lung transplant know too well what a bronchoscopy is. For the ones on the waiting list who never had one; don't worry you will get to know. A bronchoscopy is usually done by a pulmonologist (lung doctor) and consist a small camera at the end of a long probe narrow enough to fit through the airway. It can be inserted through the nose or mouth and also through the breathing tube is the patient is still on the breathing machine (ventilator). That camera is inserted all the way down in both lungs while the patient lightly sedated. The patient usually receives what some hospitals called "conscious sedation". That basically means that you are awake enough to breath and gag but sleepy enough to think what's happening is only a bad dream.


Indications after Lung Transplant


Diagnostic


There are mainly 2 reasons doctors will do bronchoscopies after lung transplant. The first reason is for diagnostic purposes where the physician may or may not suspect something wrong but needs to find out. A bronchoscopy is done at scheduled intervals after the transplant to see if the body is rejecting the new lungs. A rejection is a sign that the body is not accepting the organ and is trying to get rid of it just like it was a bacteria. It needs immediate medical treatment once diagnosed. Biopsies will be taken from several different locations in the lungs to look for possible rejection. A biopsy consists of removing a tiny bit of lung tissue and to send it to the lab under the microscope. Usually the doctor will repeat that 6 to 8 times at each bronchoscopy depending on how the patient tolerate it and how much it bleeds. It takes about 24 hours to know if the lung biopsy showed a rejection or not.


Another diagnostic reason to do a bronch is when a patient shows some signs of lung infection and a culture needs to be performed. By performing a culture, the pulmonologist will be able to identify the bug (bacteria, virus or fungi) that has invaded the new lung and treat it accordingly. Unfortunately, a lot of lung transplant patients are infected with "superbugs" that require powerful antibiotics because they have adapted and become resistant to the weaker ones like Cipro or Levaquin.


Therapeutic


The other purpose of a bronchoscopy has therapeutic intentions. In other words it means to treat a condition found in the lungs where the patient is experiencing lung transplant complications. The main ones that we see are accumulation of thick mucus that will sometimes occlude part of the lung. The doctor will use the scope and go in and wash it out real good. The real term used for the occlusion is a mucus plug. Usually a patient feels 100% better once that issue is fixed.


A more rare complication after lung transplant is called bronchial stenosis. Stenosis means narrowing. So bronchial stenosis means that the bronchus has become narrow and it is more difficult for the air to go through. That usually happens at the site where the surgeon hooked up together the new lung with the patients' own bronchus. The scar tissue that forms makes the air passage more narrow. That condition is diagnosed during a bronchoscopy and can be treated by 2 ways that are pretty similar to what a cardiologist will do to clear blockages in a heart. The first step is to use a balloon that is inflated at the narrow spot with the goal to open it up. If it opens up and stays like that, it is good. If it closes back up, then a stent may be deployed to keep it open. Just like in your heart. The difference is it happens in the lung, not in the bloodstream.


These were the main reasons bronchoscopies are done on lung transplant patients. The number one thing that is always suspected early on after transplant regardless if the patient has any symptoms is rejection. This is why bronchoscopies are performed so routinely, to catch them early.

No one wants their child to be in pain or have to deal with an illness, short or long-term. However if the vomiting happens continually and is accompanied by diarrhea, this could easily cause dehydration, which in turn could cause other serious medical complication and side effects. After you have administered fluids and the vomiting happens only occasionally everything should be fine. Confusion and/or extreme weakness are dehydration signs and should be your clue to seek medical help usually in the form of doctor examination and possible IV fluids that will help rehydrate until the vomiting stops.

Parents especially those who are new parents tend to overreact yet there is nothing as keen as a parent's intuition so if you feel that you need to get your child to a doctor or hospital right away then do so. An error on the side of caution is always the better way to go.

Making The Army Fitness Requirements

If you want to be a commando then you are going to have to increase your fitness. When it comes to getting in shape for the army, there are three very important body movements that will make your basic training and army fitness requirements a lot easier to handle when you get there.


1. Running - Running and marching long distances with weight on your back is how the majority of army units get around. So you will be doing a lot of it. Some units have troop carriers but you will still be doing a lot of running, trench assaults and closing into a position. Marching will come in whenever a big move or long silent patrol is needed.


With both of these physical activities, doing the thing you are training for is how to get better at them. Start slow and back off when aches and pains start. Heal, get stronger and then go further and faster as soon as your body allows.


Of course while in operation or on a tour, you can't readily stop for aches and pains but while you can while training, you should.


Also take into consideration that you will be doing all this in hard bottomed boots and not soft cushioned runners like you may be used to as a civilian.


2. Push-ups - Push-ups play more of a role in your actual training during boot camp and other courses but increasing upper body pushing strength is always a useful thing to do. You never know when you will need to push anything or anyone heavy around.


Besides practical uses, push-ups are a pass/fail activity during many armies recruitment process. Start on your knees and when you reach 20, do full push-ups. Increase your numbers by working just short of muscular failure once a day.


3. Pull ups - Lifting yourself over objects will come in handy time and time again. Pull ups will most likely be done during the recruitment process depending on what country you are in as well. You can increase your ability to do pull ups by starting only doing the very top half of the movement and once you reach twenty, cut the reps and go for the full range of motion.


Climbing into a truck, getting out of a trench and getting into and through a window are all activities that will use your pull up strength. Most obstacle courses have a table top, wall and rope or cargo net as well that will require pulling strength.


Not only are the above mentioned activities used for most countries army fitness testing but getting good at these three will work your whole body and make it substantially easier for you to master many other movements as a side effect.


Remember, you may be training to simply get into the army. You may also be training to have the fitness to perform at 100% when you are actually in operations. Either way, now is the time to start and keep increasing your level of capabilities so they are there when you need them.
